Rapamune Drug Information
The drug is an immunosuppressant which blocks the activity of some blood cells such as T lymphocytes that may be instrumental in rejecting transplanted organs. In this manner it achieves optimal immunosuppression in the course of maintenance therapy with corticosteroids. Rapamune has not shown signs of nephrotoxicity. However, it is not indicated for patients with a liver or a lung transplant.
Rapamune Drug Dosage
Sirolimus is administered once daily. The initial dosage is taken as soon as possible following transplantation. The maximum daily dosage must not exceed 40 mg. With de novo renal transplant patients, the loading dose of 6 mg should precede the maintenance dose of 2 mg. For patients taking it Rapamune with cyclosporine, the loading dose is up to 15 mg on day 1. From day 2, a maintenance dose of 5 mg daily is given. With patients weighing less than 40 kg the initial dose is 1 mg; the loading dose is 3 mg. With mild or moderate renal impairment patients, the maintenance dose is reduced by approximately one third. Side Effects and Precautions for Rapamune
Common side effects of Sirolimus include drowsiness, dizziness, weakness, vomiting, insomnia, headache, pain in the back, abdominal pain, fever, diarrhea, constipation, acne, urinary tract infections, hypertension. Pregnant patients or those with a lung or liver transplant should not take the drug. Its use is contraindicated for patients taking rifabutin, rifampin, ketoconazole, astemizole, itraconazole, telithromycin, terfenadine, tacrolimus, a macrolide antibiotic or voriconazole.
